Musculoskeletal ultrasound is one of the most valuable diagnostic tools in modern orthopedic and rehabilitation care. It allows your therapist to see soft tissue structures with high resolution, assess them dynamically during movement, and immediately integrate those findings into your treatment plan — all in a single visit.

What Is Musculoskeletal Diagnostic Ultrasound?

MSK ultrasound uses high-frequency sound waves to produce real-time images of the soft tissues of the body. A small handheld probe is placed on the skin with a small amount of gel — similar to a standard medical ultrasound — and the reflected sound waves are converted into a live image on screen. There is no radiation, no enclosed machine, and no discomfort.

Unlike X-ray, which only images bone, ultrasound excels at imaging the soft tissue structures that are most commonly involved in musculoskeletal pain: tendons, muscles, ligaments, bursae, and the fluid within joints. And unlike MRI — which captures a static image in a fixed position — ultrasound can be performed dynamically, imaging the tissue as it moves, contracts, and glides through its range of motion.

Ultrasound vs. MRI — What’s the Difference?

Real-time and dynamic — ultrasound images tissue during movement; MRI captures a static snapshot. Tendon impingement, snapping hip, and dynamic instability are examples of findings that appear on ultrasound but are easily missed on MRI.
Immediate results — findings are available instantly, allowing your therapist to discuss them with you on the same visit
No wait time or referral required — get answers the same day rather than waiting weeks for an imaging appointment
No metal contraindications — ultrasound is safe for patients with pacemakers, spinal stimulators, or metal implants

Ultrasound does not replace MRI for all conditions — bone, cartilage, and certain deep joint structures are better evaluated by MRI. Your therapist will advise if additional imaging is indicated based on your findings.

What MSK Ultrasound Can Detect

Tendon Conditions

Tendinopathy — thickening, heterogeneity, and loss of normal fibrillar tendon texture
Partial and full-thickness tendon tears — particularly rotator cuff, Achilles, patellar, and quadriceps tendons
Calcific tendinitis — calcium deposits within the tendon, most common in the rotator cuff
Tendon subluxation — tendons that slip out of their normal groove with movement
Paratenonitis — inflammation of the tissue surrounding the tendon

Bursa and Soft Tissue

Bursitis — inflammation and fluid accumulation in bursae (subacromial, trochanteric, olecranon, prepatellar, retrocalcaneal)
Ganglion cysts and soft tissue masses — differentiate cystic from solid lesions
Plantar fascia — thickness and integrity of the plantar fascia for plantar fasciitis diagnosis
Nerve pathology — peripheral nerve entrapment, enlargement, or injury (carpal tunnel, cubital tunnel, Morton’s neuroma)

Muscle Conditions

Muscle strains and tears — partial or complete muscle fiber disruption
Hematoma — blood collections within muscle following injury
Myositis ossificans — abnormal bone formation within muscle after trauma
Atrophy — reduced muscle bulk from disuse, neurological injury, or chronic disease

Ligament and Joint Conditions

Ligament sprains and tears — particularly ankle, knee, and wrist ligaments accessible to ultrasound
Joint effusion — excess fluid within a joint indicating inflammation or injury
Synovial thickening — marker of inflammatory arthritis activity
Baker’s cyst — posterior knee fluid collection often associated with knee pathology

One of the most powerful aspects of musculoskeletal ultrasound is the ability to image tissue in motion. Your therapist can ask you to perform specific movements while watching the ultrasound screen in real time — revealing problems that only become apparent during activity:

Tendon impingement that occurs only with shoulder elevation
A snapping tendon or iliotibial band that shifts during hip movement
Dynamic instability of a ligament under stress
Carpal tunnel narrowing during wrist flexion
Muscle herniation that appears through a fascial defect only during contraction

What to Expect During Your Ultrasound Appointment

A musculoskeletal ultrasound at Healing Star PT is quick, comfortable, and non-invasive. You will be positioned comfortably while your therapist applies a small amount of water-based gel to the skin. The probe is then gently moved across the area of interest while the image is displayed on the monitor. There is no discomfort from the ultrasound itself.

Your therapist will explain the findings as they image, walking you through what you’re seeing on screen in plain language. The MSK ultrasound evaluation typically takes 15–30 minutes and can be performed as a standalone appointment or as part of your regular treatment session. A written summary of findings can be provided for your records or to share with your referring physician.

Is MSK Ultrasound Right for Me?

Musculoskeletal diagnostic ultrasound is appropriate for a wide range of patients and presentations. It is particularly valuable if you:

Have a suspected tendon tear or tendinopathy and want answers without waiting for an MRI
Have been told you have bursitis, a cyst, or an effusion and want to understand the extent
Are experiencing nerve symptoms (numbness, tingling) and want to evaluate the peripheral nerve
Have a history of calcific tendinitis and want to assess the current state of calcium deposits
Want to monitor the progress of a known tissue injury over the course of treatment
Require imaging guidance for a procedure such as dry needling
Cannot have an MRI due to a metal implant or claustrophobia

Do I Need a Referral?

Musculoskeletal diagnostic ultrasound at Healing Star PT does not require a physician’s referral. You can schedule a standalone MSK ultrasound evaluation or have it incorporated into your existing physical therapy care. If findings suggest a condition that requires physician follow-up, surgical evaluation, or additional imaging (such as MRI), your therapist will provide a clear written summary to facilitate that referral.
