Our Vestibular Assessment
A thorough vestibular evaluation is the foundation of effective treatment. At your first visit, your therapist will perform a detailed assessment including:
- Medical history review — onset, triggers, associated symptoms, medication review
- Positional testing — Dix-Hallpike and roll tests to diagnose BPPV and identify affected canal
- Oculomotor assessment — smooth pursuit, saccades, gaze-evoked nystagmus
- Head impulse test — assessing VOR (vestibulo-ocular reflex) gain
- Dynamic visual acuity — measuring gaze stability during head movement
- Balance and gait assessment — BERG Balance Scale, Dynamic Gait Index, functional mobility
- Cervical spine assessment — to identify cervicogenic contributions
What to Expect
Vestibular rehabilitation is different from traditional physical therapy. Sessions may feel unusual at first — some exercises intentionally provoke mild dizziness as part of the habituation process. This is normal and is how the brain learns to adapt. Your therapist will always work within your tolerance and progress at a pace appropriate for your diagnosis.
BPPV typically resolves in 1–3 sessions. Vestibular hypofunction, cervicogenic dizziness, and other conditions typically require 6–12 sessions over 4–8 weeks, with a home exercise program performed between sessions. Meniere’s disease management is ongoing and may include periodic check-ins and program updates.
Do I Need a Referral?
New Jersey is a direct-access state — you can begin vestibular physical therapy without a referral from a physician or ENT. If you have undergone recent audiological testing, imaging, or are under the care of a neurologist or otolaryngologist, please bring any relevant records to your first visit. We are happy to coordinate care with your other providers.
